Job Title:        M-Health Worker/LVN

Location:        Homeless Innovations Project (HIP), Long Beach

Reports to:     Homeless Innovations Director

Hours:             Full Time; some evenings, weekends and holidays

Salary:            $37,000 to $44,000 annually equivalent (DOE) + flexible benefits

 Supervision Responsibilities: If appropriate or assigned, may undertake supervision of RN or RNP students from local nursing schools.

Summary: The Homeless Innovations Project is a program designed to integrate primary care and mental health care services within a mobile, “street medicine”, homeless outreach and intensive case management team model.  This project is funded for three years by the Department of Mental Health in LA County, Mental Health Services Act (MHSA).  This project will target people experiencing homelessness with both severe mental health issues and chronic physical health issues who are not connected to any services. The project will utilize evidenced based outreach and engagement, harm reduction, housing first, motivational interviewing and supported employment and education practices.  The goal of the project is to inform system wide learning around the integration of primary and mental health care and project participants achieve a level of recovery from their illnesses, live in permanent housing and become full participants in the community.

 

QUALIFICATIONS: 

  • Works collaboratively within an integrated mobile team to provide street outreach, community-based case management, housing, mental health and primary care services to homeless individuals 
  • Must provide all services in the field (Village HAP, MSC, Members Home, any other location where homeless individuals congregate) 
  • Provides any needed or requested license-dependent physical or mental health service(s)
  • Health education, resources, liaison with community health providers and counseling on specific health issues 
  • Provides health care in the field to a homeless population within scope of the LVN practice
  • Dispense medication(s)/injection(s) as prescribed by MD or NP 
  • Make independent assessments to identify member needs as necessary and initiate communication and collaboration within the integrated team to provide the resources to meet those needs 
  • Develop recovery plan with each member that identifies his/her personal goals and needs 
  • Utilize evidence based practices in service delivery such as motivational interviewing, harm reduction, supported employment and education, intensive case management, housing first, and critical time intervention 
  • Be an active part of the learning in this project by participating in other trainings (DMH trainings and other sources) that introduce different interventions that could be an effective strategy with this population 
  • Provides coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such physical health care, money management, grooming and hygiene, meal preparation, public transportation and other independent living skills 
  • Mediates and advocates on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements, etc 
  • Maintains all records and documentation, utilizing various technological resources needed for program compliance and accountability in accordance with MHA standards and funding source requirements

OTHER POSITION REQUIREMENTS: 

  • Must be a licensed LVN professional. Must be able to demonstrate the ability to murture and maintain good community relations on behalf of the organizaiton.
  • Must have a LPS designation or be qualifeid in a timely manner by the LA County Department of Mental Health to write involuntary holds per section 5150 of the California Welfare and and Institutions Code.
  • Demonstrate knowlege of community resources.
  • Must funciton well in a team-oriented approach to member services.
  • Valid CPR and basic first aid certificate preferred.
  • EMT certification preferred.
  • Knowledgeable in technological and computer applicaitons.
  • Driving with current class C driver's license, a vehicle for transportation, proof of valid automobile insurance, the ability to be ensured by MHALA liabiliy insurance are conditions of employment.
  • Flexible work schedule to include some evenings or weekends, and on call cell phone services.
  • Must possess a high level of tolerance and understanding for individuals who present for services wiht urgent multiple needs, issues an dproblematic behavioral characteristics.
  • Knowlegeable of the characteristics of individuals who are substance abusing and/or dually diagnosed, and provide or coordinate appropriate level of treatment as needed.
  • This project is an innovative start up and requires the ability to be flexible and adapt ot programmatic changes as they occur and the project evolves over a 3 year period.

Hours:           Full time, 9 am to 5 pm; some evenings, weekends and holidays

Pay rate:       Equivalent to $36,000 to 40,000 annually (DOE) + Flexible benefits

Summary: Serves as the primary support and case manager for young adults with mental illness by assisting them to live as independently as possible in their chosen community.  Provides license-dependent physical and mental health services as needed.

Qualifications:   Develops a Recovery Plan and/or Personal Service Plan with each member that identifies his/her personal needs and goals. 

  • Mediates and advocates on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements etc.
  • Visits members in their place of residence, hospital or jail on an as-needed basis, in order to help them live successfully and to prevent them from becoming homeless.
  • Transports members to appointments and on errands when appropriate.
  • Oversees and monitors members’ individual and group activities both inside the program and out in the community.
  • Provides coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such as money management, grooming and hygiene, meal preparation, public transportation and other independent living skills.
  • Devises and implements strategies for outreach to members who tend to isolate themselves. Knowledgeable and practices in motivational interviewing and harm reduction.
  • Supports individuals and team goals in community integration and employment.
  • Works with Community Integration.
  • Assists members in achieving community integration and employment goals. Participates in staff and community meetings and in the day-to-day operations of the program as needed.
  • Provides any needed or requested license-dependent physical or mental health service(s). 
  • Including functioning as Head of Clinical Services. Participates in staff and community meetings and in the day-to-day operations of the program as needed including TQM.
  • Health education, resources, liaison with community health providers and counseling on specific health issues. Educate Personal Service Coordinators and aid with medication assessments.
  • Provides medication room support dispense medications/injections in agency medical room as needed. Maintain a billing average of five hours per day into the Caminar System.
  • Maintains all records and documentation needed for program compliance and accountability in accordance with MHA standards and funding source requirements.
  • Provides day-to-day oversight of medication room, including daily management of administrative tasks such as logs and other records, charting of services performed, compliance with protective technical policies and procedures, and other similar duties.

  • Must function well in a team-oriented approach to member services.
  • Knowledgeable in computer applications such as Microsoft Word, Excel and Outlook
  • Must possess a high level of tolerance and understanding for individuals who present for services with urgent multiple needs, issues and problematic behavioral characteristics.
  • Must be able to maintain an empathetic “clear-eyed detachment” for the dilemma of individuals in severe need, have a tolerance for frustration and delayed gratification, and be able to juggle creatively and successfully around a variety of barriers that keep individuals from successful personal and community living.
  • Must possess a high level of patience and tolerance for a work atmosphere that is very busy and highly unstructured.
  • Must know or very quickly become knowledgeable of the characteristics of individuals who are substance abusing and/or dually diagnosed, and have knowledge of local services available for dually diagnosed individuals.
  • Demonstrate knowledge of community resources.
  • Must function well in a team-oriented approach to member services.
  • Must have a valid CPR certificate.
