Personal Service Coordinator JR # 399

Job Title:        Personal Service Coordinator

Location:        Village, Long Beach (HAP: Drop-in Center)

Reports to:    Director of Homeless Assistance Program

Hours:           Full time, M – F 9 am to 5 pm; some evenings, weekends and holidays

Salary:          $30,000 - $32,000 (DOQ)

FLSA:            Non-Exempt

QUALIFICATIONS: 

  • Provides a welcoming atmosphere for homeless with mental illness who come to the drop-in-center in search of assistance.  Shares joint reposponsibility with co-workers/team members to set-up, maintain, monitor, correct and enhance all day-to-day aspects of center functioning        
  • Participates with co-workers and members at the center in assisting with scheduling showers, using washing machines, selecting clothing, distributing food, etc
  • Assits homelss individuals with mentally illness in locating and obtaining requested services, including housing, food, health care, substance abuse treatment, benefits and entitlements, etc. Mediates and advocates on behalf of center members when necessary to obtain and keep these services        
  • Transports members to resources and services (i.e.,shelter, social security office, etc)        
  • Depending upon the program site, structure and needs, may assist in devising and implementing strategies for and conducting outreach to hard-to-engage homeless mentally ill individuals living in shelters or on the streets of the identified community/service area
  • Assists center members in developing a Personal Service Plan that identifies/reflects the member's personal needs and goals. Works cooperatively with and makes appropriate referrals to other program and service cooridnators, as well as other outside sources, as members achiev psychiatric and residential stability        
  • Works cooperatively with co-workers/team members to help assure smooth and seamless delivery of comprehensive services to program participants. Participates in staff and community meetings and in the day-to-day ooperations of the program as needed        
  • Maintain a direct contact average of five hours per day into the HMIS System
  • Maintains all records and documentation needed for program compliance and accountability in accordance with MHA standards and funding source requirements

High school diploma or GED

Minimum 3 years experience providing psychosocial rehab related services to a disabled population.

Associate degree in a human service or related field

Minimum 2 years experience providing psychosocial rehab related services to a disabled population.

Associate degree in a specific rehabilitation related field (e.g. Certified Occupational Therapy Assistant)

Minimum 1 year experience providing psychosocial rehab related services to a disabled population.

Bachelor degree in a human service or related field

No experience required, but preferred.

 

OTHER POSITION REQUIREMENTS: 

  • Must function well in a team-oriented approach to member services.        
  • Knowledgeable in computer applications such as Microsoft Word, Excel and Outlook.
  • Must possess a high level of tolerance and understanding for individuals who present for services wiht urgent multiple needs, issues and probelmatic behavioral characteristics.        
  • Must be able to maintain an empathetic "clear-eyed detachment" for the dilemma of individuals in severe need, have a tolerance for frustration and delayed gratification, and be able to juggle creatively and successfully around a variety of barries that keep individuals from successful persona and community living.
  • Must prossess a high level of patience and tolerance for a work atmosphere that is very busy and hightly unstructured.
  • Must know or very quickly become knowledgeable of the characteristics of individuals who are substance abusing and/or dually diagnosed, and have knowledge of local services available for dually diagnosed individuals.
  • Valid CPR certificate prferred.
  • Driving with current class C driver’s license, a vehicle for transportation, proof of valid automobile insurance, the ability to be ensured by MHA liability insurance are conditions of employment.

Summary: Serves as the primary support and case manager for adults with mental illness by assisting them to live as independently as possible in their chosen community.

  • Provides coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such as money management, grooming and hygiene, meal preparation, public transportation and other independent living skills
  • Devises and implements strategies for outreach to members who tend to isolate themselves
  • Knowledgeable and practices in motivational interviewing and harm reduction
  • Participates in staff and community meetings and in the day-to-day operations of the program as needed
  • Maintain a billing average of five hours per day into the Caminar System
  • Maintains all records and documentation needed for program compliance and accountability in accordance with MHA standards and funding source requirements

  • Participate in program designed for Licensed Waivered staff to attain supervised hours and be willing to pursue Licensed Clinical Social Worker status.
  • Participate in program designed for MFT intern staff to attain supervised hours and be willing to pursue MFT Licensed status.
